Transitional Services
The Bridge Between the System and a Stable Life.
Dunamis Transitional Services wraps around young people as they move through — and out of — the system. Housing, education, employment, family, and long-term aftercare. All connected. All built around the individual.
Transition Is Not a Moment. It Is a Process.
For most young people leaving the justice system or foster care, the hardest part is not the programme — it is what comes after. The moment the formal support ends is often the moment the risk returns.
Dunamis Transitional Services are designed to close that gap. We provide structured, sustained support across four interconnected domains — housing, education and employment, family reconnection, and aftercare — so that the transition from system involvement to independent living is not a cliff edge, but a bridge.
Every transitional service is tied to the young person's individual reentry plan, built from day one of their journey with us.
"For I will restore health to you and heal you of your wounds, says the Lord."
Housing Stability
We assess housing needs from day one and work with young people to secure safe, stable accommodation — transitional, supported, or independent. We provide tenancy support and independent living skills so housing is not just found, but kept.
- Housing needs assessment and planning from day one
- Transitional and supported accommodation referrals
- Independent living skills development
- Tenancy support and landlord liaison
- Emergency housing response
Education & Employment
We support young people to re-engage with education and enter sustainable employment — from GED pathways and vocational training to job placement and financial literacy. We remove barriers and build the skills that make employment stick.
- Education pathway assessment and re-enrolment support
- GED, vocational, and alternative education pathways
- Job readiness and employment skills training
- Job placement and employer connections
- Financial literacy and benefits navigation
Family Reconnection
We work with young people and their families to rebuild relationships where it is safe and beneficial — and to build alternative support networks where it is not. Family engagement is structured, supported, and always centred on the young person's wellbeing.
- Family relationship assessment
- Mediation and facilitated conversations
- Family engagement and education
- Alternative support network building
- Ongoing family liaison and support
Aftercare & Long-Term Support
Our support does not end when the programme does. We provide long-term aftercare — regular check-ins, crisis response, mentorship, and ongoing connection — so that the gains made during the programme are protected and built upon.
- Regular post-programme check-ins
- Crisis response and re-engagement
- Ongoing mentorship and peer support
- Benefits and entitlements monitoring
- Long-term outcome tracking
Stage 6 of Our Model
Aftercare Is Not an Add-On. It Is the Final Stage.
Most programmes end when the young person leaves. Ours does not. Stage 6 — Aftercare — is the final and ongoing stage of our 18–24 month model. It is the structured commitment to staying connected with young people after they have completed the program.
Aftercare includes regular check-ins, crisis response, mentorship, peer support, and long-term outcome tracking. We stay connected — because the research is clear that the first 12–24 months after leaving the system are the highest-risk period for re-involvement.
